Job Summary
You will provide person-centred care coordination for clients living with complex aged and healthcare needs, whilst also utilising discipline specific skills to support each client to improve their health and wellbeing.
This position is under the Health Independence Program (HIP) and will be part of the HIP Complex Care and Chronic Disease Team focused on improving health outcomes of people at risk of hospital presentation.

Key Requirements
Current AHPRA registration if required
Current Victorian Driver’s License
Experience with aged clients with chronic diseases
Current National criminal history check or willingness to obtain
