Job Summary

  • This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.
  • Coordinate care for long-term care residents using Ennoble Care’s EMR, maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions.
  • Ennoble Care offers a variety of programs including, remote patient monitoring, behavioral health management, and chronic care management, to ensure that our patients receive the highest quality of care by a team they know and trust.

Key Requirements

  • Active, unrestricted clinical certification or licensure (PCT, CNA, MA, LPN, RN, or comparable)
  • Minimum 1 year patient-facing experience in long-term care
  • Comfortable communicating proactively via Microsoft Teams, email, phone, and in person
  • Proficiency with Microsoft 365
  • Valid, unencumbered driver’s license and reliable transportation
  • Ability to pass a background check and drug screen
