Job Summary

  • The Senior Patient Navigator provides support and guidance throughout the patient’s care continuum by helping patients navigate through the maze of doctors’ offices, clinics, hospitals, outpatient centers, insurance and payment systems.
  • Ensures patients have access to insurance and social service programs for which they are eligible and maintains knowledge of insurance referral requirements.
  • Assists lower-level Patient Navigators as needed and acts as liaison/advocate for patients, physicians, and staff in facilitating ease of care.
