This role is responsible for assisting in the planning, developing, and directing of social service programs to meet the medically related emotional and social needs of residents.
The position requires coordinating social service activities with other departments and obtaining resources from community social, health, and welfare agencies.
Employees must maintain strict confidentiality of all resident care information including protected health information while reporting suspected fraud or unauthorized disclosures.

Key Requirements
Bachelor's Degree in Social Work (BSW)
One year supervised social work experience in healthcare
State social work licensure (LSW, LMSW, LCSW) if required
Completion of mandatory in-service trainings including HIPAA
Prior long-term care or skilled nursing facility experience preferred
