The ideal candidate should have experience in human services or a related field and will be responsible for patient education, monitoring, and referrals while engaging with healthcare professionals
Job Summary
The Care Navigator functions as a core member of an Integrated Care team that includes the Primary Care Provider (PCP), Behavioral Health Clinician (BHC), and psychiatric provider.
The Care Navigator is responsible for coordinating and supporting Mental Health care within the clinic and for coordinating referrals to clinically indicated services within and outside of the clinic.
This role will report to the Manager, Behavioral Health/Primary Care Integration or designee.
