Job Summary

  • Conducts and documents comprehensive assessment of client and/or client’s needs and participates in development of client’s service plan, consistent with assessment.
  • Monitors client’s progress by making referrals, tracking client’s appointments, performing follow-up on services rendered, and performing periodic reassessments of client’s changing needs.
  • Provides case consultation (i. e., consulting with service providers/collaterals in determining client’s status and progress).

Key Requirements

  • Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or related field
  • Minimum of zero years experience in case management/service coordination
  • Working knowledge of assigned population group
  • Working knowledge of SCS programs and services
  • Working knowledge of applicable regulations
  • Good interpersonal skills
  • Written and oral communication skills
  • Reliable transportation for frequent travel
  • Active Driver's License
