Key Requirements

  • MD, DO, or equivalent
  • Board certification/eligibility in Allergy-Immunology or Rheumatology
  • Completion of ACGME-approved pediatric residency
  • Completion of accredited fellowship program
  • Wisconsin medical license
  • Eligible for billing provider enrollment and hospital privileges
  • Must pass initial caregiver check
