Job Summary
This role is responsible for assisting in the planning, developing, and directing of social service programs to meet the medically related emotional and social needs of residents.
The incumbent must assist residents in achieving independence while providing consultation to solve problems through the development of social service programs.
Employees are required to maintain strict confidentiality of all resident care information including protected health information and report any suspected fraud.

Key Requirements
Bachelor's Degree in Social Work or human services field
Minimum one year supervised social work experience in healthcare setting
Social work licensure (LSW, LMSW, LCSW) if required by state law
Completion of mandatory in-service trainings including HIPAA and abuse prevention
