Job Summary
The Navigator partners with patients, families, the interdisciplinary team, and community resources to provide well-coordinated, timely, compassionate, interdisciplinary care.
Utilizes the nursing process to perform assessment of patients” clinical, emotional, spiritual, psychosocial needs serving as a single point of contact throughout the care process.
The Navigator is responsible for ensuring all patients referred with a cancer related diagnosis or suspicious finding receive effective and comprehensive services.
