This role provides oversight on facilities' weekly PDPM meetings and tracks the effective utilization of PDPM tools for skilled patients.
The position requires assessing new admissions, determining their level of care, and ensuring accurate completion of all MDS assessments within regulatory timeframes.
Candidates must coordinate interdisciplinary participation in completing Minimum Data Sets and assist disciplines in formulating comprehensive resident care plans.
