Job Summary

• You’ll work remotely, collaborating directly with care teams and practicing under a Collaborative Practice Agreement (CPA) to conduct virtual patient assessments, order and monitor evidence-based treatment plans, optimize therapy, and resolve medication-related problems.
• Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments aligned with evidence‑based standards.
• Receive a $10,000 Sign-on Bonus, salary plus bonus, medical, dental, and vision insurance starting day one, 401(k) with match, 4 weeks PTO, and 10 paid holidays.
