Job Summary
The APP would hospital follow up patients with a heavy emphasis on post-acute stroke care.
The APP will participate in the education of learners (other APPs, students/APP fellows, nurses, and new clinical teammates) and with our operational leaders and stroke program coordinator to help support transition of care initiatives.

Key Requirements
2+ years of experience as APP
