Aabana is seeking a Clinical Informaticist for their Home Healthcare and Hospice division, primarily working remotely in Illinois and Wisconsin. The role focuses on providing expert support for electronic health records (EHR), enhancing clinical workflows, and collaborating with clinical and IT teams to improve patient care
Job Summary
Serve as a subject matter expert for the EHR and provide in-depth support to clinicians in the outpatient setting, including physicians, advanced practice providers, nurses, medical assistants and other clinical support staff, and serves as a liaison between clinicians, operational leadership and IT.
Leverage clinical data analytics to drive informed decisions and create actionable insights through dashboards and reports.
Participate in cross-functional teams in reimagining clinical process and pioneer novel approaches to enhance patient care, teammate satisfaction and safety and outcomes.
