As part of the Connected Care team, the Advanced Practice Provider will assess, diagnose, and manage both acute and chronic medical conditions while collaborating with an interdisciplinary care team.
New graduates with prior SNF or long-term care experience are encouraged to apply for this day-shift position with a rotating call schedule.
