Must-have
Current LPN/LVN license required

Perform resident assessments for condition changes
Maintain regulatory documentation compliance
Nice-to-have
3-5 years long term care experience preferred
Knowledge about dementia highly desirable
Acute or skilled nursing community experience
Excellent written and verbal communication skills
Working knowledge of Point of Care systems
Key Requirements
Current LPN/LVN license
Minimum 2 years long term care experience
Current First Aid Training and Certification
Completion of background check including drug screen
