• Associate's Degree Nursing required
• Bachelor's Degree Nursing preferred
• Active Registered Nurse Licensure
• 2+ years ambulatory care experience
• 2+ years practice based case management experience
• 2+ years primary care case management experience
• 2+ years specialty case management experience
• Home care experience strongly preferred
• Hospital case management preferred
• Discharge planning experience preferred
• Experience using EPIC preferred
• Experience working at primary care practice preferred
