Collaborate with multidisciplinary caregivers to provide comprehensive care coordination and disease management for patients with chronic conditions, with a focus on IBD and UC.
Monitor hospital inpatients, ensure timely follow-up visits, and manage the completion of tests, procedures, imaging, and genetic results to support providers in delivering optimal treatment.
Assist with procedures in the Motility Lab and work collaboratively with providers in the Gastroenterology and Colorectal Surgery departments to triage patient concerns and direct care effectively.
