Houston Transitional Care is seeking a Remote RN MDS Coordinator to manage patient assessments, coordinate care plans in line with regulatory requirements, and oversee nursing staff in a long-term care facility. The ideal candidate will possess a current nursing license, strong organizational skills, and the ability to lead effectively in a remote setting

Job Summary

  • Administer patient assessments, oversee the assessment process, setting the assessment schedules and assuring that assessments are done in an accurate and timely manner.
  • Direct the day to day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long term care facility.
  • Participate in developing, planning, conducting, and scheduling in service training classes that provide instructions on “how to do the job,” and ensure a well educated nursing service department.

Key Requirements
