Job Summary
As a valued member of our clinical team, you will provide direct comprehensive care to LTC facility residents with specialized and complex needs based on enhanced clinical knowledge and skills.
Performs a person-centered health assessment of residents on admission, annually and as needed (including quarterly medication reviews) to develop a person-centered plan of care while collaborating with resident, family interdisciplinary team and other health professionals.
