  • Responsibilities include conducting initial assessments, maintaining patient eligibility records, managing intake and enrollment, and making timely referrals to Part B and other supportive services to ensure continuity of care.
  • Build and sustain relationships with community partners and stakeholders to expand resource access and enhance patient outcomes.
