Provide leadership, supervision, and support to a team of assigned staff providing services to consumers enrolled in Tennessee Health Link.
Ensure consumers are seen upon admission and discharge from hospitals, diversion services, or residential treatment facilities, and that their daily needs are planned for and coordinated.
