Job Summary
This role provides essential emotional and spiritual support to patients and families facing life-limiting illness while honoring their unique beliefs and values.
The Hospice Chaplain coordinates rituals, prayers, and ceremonies as requested by patients and collaborates with the interdisciplinary care team to develop individualized care plans.
Candidates must maintain accurate documentation of spiritual assessments and interventions while upholding strict confidentiality and ethical guidelines.
