Key Requirements

  • PharmD from ACPE accredited school
  • Completion of PGY1 and PGY2 residency or 5 years clinical experience
  • TX Pharmacist license or eligible
  • CPR – BLS certification
  • BPS certification within 1 year
  • Preceptor license required within 90 days
