The Behavioral Health Consultant (BHC) provides supportive advocacy to patients and their families as part of an interdisciplinary health care team to overcome barriers to progression-of-care.
The BHC works with chronically ill patients to assist them with management of the behaviors affecting their health.

Key Requirements
