Job Summary

• The Social Worker (Grade 2) works as a member of a multi-disciplinary team (MDT) that provides, quality patient –centered nursing care and advocate for and facilitate the involvement of individuals, their families and significant others in planning and evaluating care.
• The aim is to work towards recovery and improved health outcomes through assessment, planning, implementation and evaluation of clinical interventions ensuring a holistic approach to the client, their family and carers.
