Therapists will work independently to assess and address patient mobility, safety, and equipment needs within their own homes.
The role requires extensive travel across Chicago North Side areas including Irving Park, Albany Park, Uptown, and Lakeview with rotating weekends and holidays.

Salary
Base: $44.15 - $66.25 hourly ($86,320 - $129,376 annual); Bonus: Up to $10,000 sign-on bonus; Benefits: Comprehensive package including incentive pay, tuition assistance, and mileage reimbursement
Skills & Requirements
Must-have
Licensed Physical Therapist in Illinois
1 year clinical experience required
Proficient with home therapy equipment
Ability to lift up to 35 pounds
Willingness to travel extensively
Availability for weekend and holiday rotation
Nice-to-have
Strong interpersonal communication skills
Time management and self-organization
Experience with computer-based documentation systems
Commitment to quality cost-effective care
Ability to work independently as a case manager
Key Requirements
Illinois Physical Therapy License or eligibility
Minimum 1 year of PT clinical experience
Must be an external candidate for bonus eligibility
