  • Pair Team is reimagining how Medicaid and Medicare serves underserved populations by delivering whole-person care through a tech-enabled medical group.
  • The Lead Care Manager will engage with individuals experiencing Serious Mental Illness/Substance Use Disorder, homelessness, or high medical needs, building trust and providing comprehensive support.
  • This role primarily allows for remote work but includes 2-3 on-site visits per month in the community, close to the employee's city.
