Responsible for optimizing patient care by promoting rational, safe, and cost-effective drug therapy.
Initiates and documents pharmacist interventions to change prescribing where required and participates in patient care rounds as appropriate.
Contributes to and participates in the departmental medication use evaluation plan, including but not limited to CMI (Clinical Management Initiatives).
