Job Summary
Case Worker coordinates services for respective population, including providing information about professional development opportunities, quality improvement strategies, and counseling to improve client’s social and psychological conditions.
The Case Worker will assist patients with their tobacco cessation efforts and will educate health care providers on tobacco cessation best practices, and the use of appropriate pharmacotherapy products.

Key Requirements
Bachelor's degree in relevant field
Minimum 1 year of relevant experience

Bachelor’s degree required in Health/Mental health related field
Significant experience in tobacco prevention and cessation
