  • The Patient Care Resource Manager is a nurse case manager that facilitates patients through the health care system from pre-admission through post discharge coordination of care, resources and services utilizing the nursing process.
  • The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions and throughput and to enhance care quality and efficiency and ensure efficient resource utilization.
  • The PCRM collaborates with physicians, nurses and other health care workers to manage the patient transition to the next level of care and is the central information source for third party payers.
