Job Summary
The Social Worker (MSW) is a non-clinical social work position responsible for discharge planning, case management, and psychosocial support of patients and their families within the hospital setting.
The Social Worker also provides advice or guidance for any other non-medical problem or barriers to care patients may have that may hinder their rate of recovery or delay discharge and serves as a resource for continuity of care options after discharge.
