Key Requirements

  • Level II: Medical Technologist registration + 4 years experience OR Bachelor's degree + Oklahoma medical practitioner license + 4 years experience OR LPN license + 6 years experience OR RN license + 5 years experience OR BSN + 4 years RN experience OR Social Worker license + 4 years experience
  • Level III: Medical Technologist registration + 6 years experience OR Bachelor's degree + Oklahoma medical practitioner license + 6 years experience OR LPN license + 8 years experience OR RN license + 7 years experience OR BSN + 6 years RN experience OR Social Worker license + 6 years experience
  • Must complete Long Term Care Surveyor training and certification within one year of hire
  • Must be legally authorized to work in the United States without visa sponsorship
