Bachelor's degree in Physical Therapy (prior to 2002)
Master's degree in Physical Therapy (prior to 2016)
Doctorate degree in Physical Therapy
Licensed Physical Therapist by Arkansas State Board
2 years of experience in Physical Therapy
Proof of legal authority to work in the United States
