Job Summary
Under guidance of supervisor, the Social Worker I (MSW) assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes.
This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge.
Serves as liaison between medical and nursing staffs, patients, relatives and appropriate outside agencies.
