Perform skilled nursing assessments
Coordinate interdisciplinary communication

Key Requirements
Graduate of accredited School of Nursing
Current RN license in good standing
Current CPR certification
New graduates welcome with prior LVN experience
Previous skilled nursing or long-term care experience preferred
