Your role is to provide efficient and effective integrated person-centred health and social care to residents residing in SingHealth region.
You will serve as a touchpoint to SingHealth residents and patients, and journey with them towards better health.
You will support Senior Associate Wellbeing Coordinators and Wellbeing Coordinators in identifying and carrying out interventions from residents’ care plans.

Key Requirements
Local Diploma in healthcare or non-healthcare studies
Higher NITEC / NITEC / GCE N/O/A Level with 2 years relevant experience
