Job Summary

  • Establishes course of action by exploring options and setting goals with residents and their families to assist them in achieving the highest practicable level of self-care, independence, and well-being.
  • Provides medically related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident, including emotional support and assistance with coping mechanisms.
  • Develops and maintains strong working relationships with internal departments and external community/health/welfare agencies to secure resources and coordinate services, including discharge planning and outpatient appointments.
