Establishes course of action by exploring options and setting goals with residents and their families to assist them in achieving the highest practicable level of self-care, independence, and well-being.
Provides medically related social services, emotional support, and addresses emotional problems, including grief related to loss, to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident.
Assists residents with healthcare decisions, personal shopping, inventory of belongings, securing community resources, obtaining prosthetics, and coordinating discharge planning with appropriate agencies.
