Job Summary
The Childbirth Center Discharge Coordinator will participate in boost rounds with pediatric, obstetric, lactation, care management and neonatology teams and facilitate discharge needs for all mother/infant dyads on the Childbirth unit.
This position is grant-funded and requires a minimum of 2 years RN experience, working 40 hours weekly on Days.
Responsibilities include ensuring referrals are scheduled, coordinating follow-up appointments, facilitating priority discharges, and acting as a liaison for various support services.
