The position requires coordinating referrals, developing community service plans, and communicating with third-party payers for reimbursement authorization.
Candidates will work in a hybrid model with time spent in the clinic, in the field, and remotely.

Skills & Requirements
Must-have
Active RN license in Massachusetts
2+ years ambulatory or case management experience
EPIC electronic health record proficiency
Nice-to-have
Home care experience preferred
Spanish language skills for patient communication
Experience with dual diagnosis conditions
Key Requirements
Associate's Degree in Nursing required
Bachelor's Degree in Nursing (BSN) preferred
Active Registered Nurse licensure for Massachusetts
2+ years of ambulatory care or case management experience
