The primary purpose of this job position is to assist in planning, developing, organizing, implementing, evaluating, and directing facility's social service programs to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis

Job Summary

  • The primary purpose of this job position is to assist in planning, developing, organizing, implementing, evaluating, and directing facility's social service programs to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
  • Essential duties include meeting with administration, medical and nursing staff, and other related departments in planning social services, as well as developing and implementing policies and procedures for identifying resident needs.
  • The role involves assisting residents in achieving the highest practicable level of self-care, independence and well-being, and providing medically related social services to attain or maintain physical, mental and psychosocial well-being.

Key Requirements

  • High school diploma or equivalent

  • Ability to apply mathematical concepts
  • Ability to solve practical problems
  • Ability to interpret instructions
  • Must occasionally lift and/or move up to 25 pounds
