  • Partner with Social Workers, Nurse Care Coordinators, patients, and families to address patients’ progression of care.
  • Assist in identifying, researching, explaining, and arranging resources such as transportation, housing, and financial assistance.
  • Provide direct patient care by phone, conduct behavioral health screenings, and assist with safe patient transitions.
