Key Requirements

  • Bachelor's degree in Nursing or Master's in Social Work/Healthcare Administration
  • Minimum 5 years of case management experience in Skilled Nursing
  • 2-3 years in a regional or multi-site supervisory role
  • Active unrestricted RN, LPN/LVN, or LCSW/LISW license
  • Deep understanding of PDPM, Medicare Part A & B, and Medicaid
