RN Case Managers work collaboratively with inter-professional teams across the continuum of care to facilitate and ensure effective transitional care coordination.
A caregiver in this position works a minimum of 2 weekend days per month from 8:00 AM – 4:30PM, including one major and one minor holiday per year.

Key Requirements
Graduate from an accredited school of professional nursing (ADN or BSN program)
Current state licensure as a Registered Nurse (RN)
Basic Life Support (BLS) certification
Three years of recent RN clinical experience
Bachelor of Science in Nursing (BSN) preferred
Certification by an approved entity within one year of eligibility preferred
Two years of recent experience in utilization review, quality or care management preferred
