Job Summary

  • The Care Advocate Team provides commercial members with high-risk complex care management support, collaborating with a multidisciplinary team to keep members in optimal health while reducing costs.
  • As the High-Risk RN Case Manager, you will case manage the commercial population to support their health needs, working alongside a LMSW, Pharmacy Team, and Registered Dietitian.
  • This role requires telephonic, home visits, and televisit platforms, and is hybrid with a few in-person meetings and home visits as necessary.
