Kern River Transitional Care is seeking a Full-Time MDS Assistant (LVN) to oversee and manage MDS assessments and PDPM tools for skilled patients in a remote work environment. The ideal candidate should have a strong nursing background, experience in long-term care, and excellent communication skills

Job Summary

  • Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks facility effective utilization of PDPM tools.
  • Tracks and monitors QM and QASP performance monthly and quarterly, providing data analysis and clinical recommendations.
  • Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change, and coordinates interdisciplinary participation.
