Works with multidisciplinary patient care team and patient/caregivers to provide optimal pharmaceutical care, providing age-appropriate care to patients from birth through 21 years and select adult patient populations.
Serves as a pharmacotherapy expert to prospectively recommend and evaluate individualized drug therapy, providing therapeutic drug monitoring and recommending evidence-based patient-specific actions.
Participates in committees related to medication safety and quality improvement initiatives and demonstrates pharmacy practice leadership.

Key Requirements
Bachelor’s Degree from an accredited college of pharmacy, required
