Key Requirements
Bachelor's Degree in Nursing or Social Work
Valid RN, LVN, or LPN license in state
Two years clinical nursing experience preferred
Knowledge of Medicare, Medi-Cal, and Medicaid programs
Certificate as Certified Case Manager (CCM) is a plus
