Key Requirements

  • social worker or licensed social worker with 8 years experience
  • bachelor's degree in public health, hospital/public administration, behavioral, environmental, or fire safety and 10 years experience

  • Surveyor Minimum Qualifications Test (SMQT) certification
  • successful completion of Federal/State training for Nursing Facilities
  • successful completion of Surveyor Minimum Qualification Training (SMQT) within one year of hire
