Job Summary
Provides oversight on facilities’ weekly PDPM meetings for skilled patients and tracks/monitors facility effective utilization of PDPM tools.
Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change, and coordinates interdisciplinary participation.
Assists disciplines in formulating and revising care plans, ensuring resident problems are identified, goals established, and nursing intervention is appropriate.
